Our providers work by appointments, which can be made by phone or in person. While every effort is made to ensure that you are seen at your scheduled time, all appointment times are approximate as individual patients with the same problem can require different amounts of time. Also, unfortunately, emergencies do occur and surgeries can run longer than expected, thus, delays are often unavoidable. Your patience is appreciated.
Cancellations
If you need to cancel or reschedule your appointment, please notify us as far in advance as possible. This allows for time to accommodate other patients. Regrettably, emergencies will occasionally require us to reschedule your appointment with little or no notice.
Initial Visit
Please arrive early for this visit, as you will be asked to complete a Patient Information Form as well as a Medical History Form. Please be sure to bring your Health Insurance card(s), Photo ID and any medical records or x-rays pertaining to your condition. These forms may be filled out before your appointment by compleating the downloaded PDF form on our website under the heading "Forms".
